How?
This is a health screening test. A physician's order is not required.
To minimize waiting time, call the Appointment Desk at 774-5693 to schedule a day and time to come in for testing. On the day of your appointment, you should stop at the Reception Desk in Foust 200. Bring your CMU picture ID or other picture identification with you.
If you come in as a walk-in patient, you will need to stop at the Primary Care Suite Appointment Desk, Foust 200 and complete the pre-registration process there.

Fasting required. Nothing to eat or drink, other than water, for 12 hours before having blood drawn
