File A Report
A report may be submitted to the Office of Student Conduct by any member of the University Community. To file a report, click on the link below. You will be asked to enter information pertaining to the incident you are reporting. You may file an anonymous report, however, without providing your name and contact information it may be very difficult to investigate the incident you are reporting.

CMU Faculty, Staff, Students, and the public should use this link to report any possible violation of The Code of Students Rights, Responsibilities, and Disciplinary Procedures (The Code of Conduct) i.e. Academic Integrity, Classroom disruptions, Alcohol, Drugs, Fights, Threats, Hazing etc.
File a Sexual and Gender-based Misconduct Report
Report a concern about anything you think might be related to the CMU Sexual and Gender-Based Misconduct Policy (i.e. dating violence, domestic/intimate partner violence, sexual assault, sexual exploitation, sexual harassment, or stalking).
