Knowledge Services Forms

Journal Article Request - This form is to be used for requesting full-text articles from the Knowledge Services Department.

Library Card Access Registration Form - This form is to be used to request a library card. The library card allows you to check out materials from the Knowledge Services Department, as well as obtain access to electronic subscription resources on our website. 

Library Literature Search - This form is to be used for requesting a literature search from the Knowledge Services Department.  Please be as specific as possible; state points to be included as well as those to be excluded. 

Library Resource Recommendation Form - This form is to be used for the recommend purchase of a book, electronic resource, journal subscription, or media resource that Knowledge Services does not currently own.  Please give us as much detail as you can so that we may quickly process your request.  

Library Survey - This form seeks input from you about the library and our services. Your feedback is essential and will assist with decision-making and performance improvements. Thank you in advance for your assistance.
