Independent Contractors
Independent contractor services require a signed contract/agreement to be in place prior to the services commencing. Prior to hiring an individual (sole proprietor/DBA) to perform a service, departments must complete and submit the Independent Contractor Questionnaire. If the individual is determined to be an independent contractor and not an employee, departments shall complete one of the IC form contracts and have it signed by the IC. If the services will be performed by a Corporation, partnership or LLC, departments shall work with Contracting and Purchasing Services to develop a contract or issue a purchase order. If the department is aware that they are hiring a DBA, corporation, or LLC of a CMU employee you must check the Conflict of Interest Report (Cmich login) to verify a disclosure statement is on file. Note: In all cases, the contract must be signed by a CMU employee with appropriate contracting authority.
Independent Contractor Agreements that are over $4,999.99 require a bid exemption form to be completed and approved. If your agreement will be over $4,999.99, please complete the bid exemption form found below. You will receive a notification when the bid exemption has been approved and then you may move forward with completing the Independent Contractor Agreement.

Speaker, presenter, performer, program reviewer or athletic official
Complete the form required to hire an independent contractor for the purpose of acting as a speaker, presenter, performer, program reviewer or athletic official. The required form will open in a new window. Once you have completed that form you will be able to print a completed contract.

After the services have been rendered, please park the contract and any related vendor documents for payment. If you have any questions related to vendor payments please contact payables@cmich.edu.
All others individuals
Complete the Independent Contractor Questionnaire which is required to hire an independent contractor for performing a service. This independent contractor will not be a speaker, presenter, performer, program reviewer or athletic official.
The required form will open in a new window. Once you have completed the Questionnaire you will click submit and it will be routed to an HR Representative. Once the questionnaire is approved by HR, you will receive an email with a link to the contract template. After the service has been completed, please park the contract for payment.
